Initial registrants should come personally to the PRC Central Office or to the nearest Regional Office to file his/her application for registration and to affix his/her signature in the registry book.
All Registration Certificates and PRC ID shall be claimed by the license personally at the Central Office or the Regional Office where the application for the same has been filed. If there is an unavoidable need for a representative, he must present his/her identification paper with picture and a Special Power of Attorney.
Pursuant to Section 16, of Republic Act No. 9173, “all successful candidates in the examination shall be required to take an oath of professional before the Board or any government official authorized to administer oaths prior to entering upon the nursing practice”.
Requirements
- Duly accomplished Oath Form or Panunumpa ng Propesyonal
- 2012 Community Tax Certificate (Cedula);
- Two (2) pieces passport-size pictures (colored with white background and nametag);
- One (1) piece 1”x1” picture (colored with white background and nametag);
- One (1) piece documentary stamp;
- Original and photocopy of NSO Birth Certificate (original copy for presentation purposes only); and
- Initial Registration Fee of P600.00 and Annual Registration Fee of P450.00 for 2012-2015.
