RN Heals 4 Requirements for East Avenue Medical Center 

1. Letter of intent 
2. Updated Resume 
3. Ceriticate of Good Moral Character -or- NBI Clearance 
4. Diploma and Transcript of Records (Certified True Copy only) 
5. Certificate of Board Rating (can be obtained at the PRC office) 
6. Certificate of trainings and seminars after graduation 

NOTE: All Documents must be certified true copy ONLY
