1. Apply for CES Report either in CGFNS or IERF. I highly suggest you process it with CGFNS online, HP&S Report ($328) would suffice. See step by step instructions here. CGFNS will then forward your CES report directly to Vermont; they will also furnish you a copy.
2. Download the latest Vermont NCLEX Application forms and instructions (8 pages) for foreign nurses here.The first 3 pages are the instructions on how to fill up the form. PDF reader required to open file.
3. Fill up pages 4-8 (page numbered 1-5) to the best of your knowledge. Paste your 2×2 picture on the space provided in page 4. You will also include in this packet your photocopy of license (front and back in one page) certified by PRC, photocopy of your passport (face page), $150 Application fee in bank draft, money order, etc payable to ‘Vermont Secretary of State.’
Checklist for Vermont NCLEX Application:
- Included the $150 fee
- Included a 2×2 inch photo
- Included email address – Printed CLEARLY.
- Filled out educational information
- Filled out work history information
- Filled out license and passport information
- Answered question concerning whether or not you have taken the NCLEX, and how many times.
- If you have taken the NCLEX, you have included copies of your fail letters from the Board of Nursing you took the exam through
Included copy of passport - Included copy of initial nursing license. If you hold any additional active nursing licenses, include copies.
- Answered ALL legal questions
- Signed application
- Requested CES report from either IERF or CGFNS and indicated its status on the application.
